New Patient Forms

Please have all forms filled out as thoroughly as possible. If there is any lab work or doctor's notes that you would like Dr. Vinitsky to review, please make copies and bring them into your first visit.

Dr. Vinitsky sees patients who are chemically sensitive. Please REFRAIN from wearing any fragrances!!!!

If new patient appointments are NOT cancelled within a 48 hour period of your actual appointment time, then you WILL be charged $100 for a missed appointment. 

When all forms are filled out to the best of your ability, please send them to the office atleast a week in advance to ensure us you intend to come for your consultation. 

 When making an appointment, a credit card will be required for a $100 deposit to hold your appointment time.

Instead of a quick in and out visit, you’ll notice the difference immediately. Our staff and Dr.Vinitsky LISTEN to YOU.  They take time to understand what’s happening to you! An in-depth interview is enhanced by your participation in providing details of your personal Accordion Reserve.^©  A thorough examination follows.  A set of working diagnoses is formulated.  Then a comprehensive set of tests will be recommended. Depending on timing of your visit, individual needs and your interest in pursuing these recommendations, some tests will be performed on the same day.

You may be coming for a second opinion or may have some expectations about initiating or changing treatment for your medical condition. If you have already had recent extensive testing, new testing recommendations will be adjusted accordingly.  Treatment recommendations are always based on sound clinical judgment after a thorough consideration of test results as they relate to your interview and examination.  Therefore, a follow-up consultation is most often appropriate before a substantial change in treatment is recommended.
